I have noticed this on several sempervivum I have, which are not related, not even grown in the same location, all in good health.
There is a single leaf that is growing at an angle of 45deg.

Has anyone seen this as a pattern with sempervivum, with just a single leaf?

These specimens are around 4cm (1.6inch), the same growth is present on a much larger specimen which is around 13cm (5inch) that I don't have a photo of.

I have not noticed that on any of mine, but that doesn't mean that I don't have some showing that growth habit. I will watch out for it when I am working out in the beds.
Your semps look healthy. Do you grow them outdoors, or indoors under lights?

All grown outside in pots.
I attempted to grow some indoors (without artificial lighting) and the results were from not interesting to bad, some specimens that experienced cold overwinter turned out to finally to have such a nice shape.

I agree Paleo, going through an outdoor cold spell brings out the beauty in them.

Yes I agree too, they kind of close up and get puffy or something
