Since the process is not new to you, there must be something different happening...
-- more or less watering
-- different water source
-- large temperature difference
-- more or less light
-- different part of the house
-- different fertilizing
-- different bag of orchid bark
-- etc.
If you can't come up with any answers, maybe your orchid bark became contaminated with a pathogen.
