Get it out of the decorative pot. I'll bet that it does not have drainage holes and could be the reason that your orchid is staying too wet.
Orchid roots drown under those conditions. Get it repotted into fresh orchid media, NOT SOIL as everyone else has said.
Read all of our forms regarding Phalaenopsis, it will recover.
